Exercise your rights

You can file your application for exercising rights also in a free format. In this case, you will have to include the following information in it:

• Your names
• PIN 
• Date and place of birth (if you are not a Bulgarian citizen)
• Address of correspondence*:
• ID card number, date of issue, validity and its issuing authority
• Address of residence and address of correspondence if different;
• Phone number 
• Email:
• How you would like to exercise your rights under EU Regulation 2016/679: in person or by an explicitly authorized person (in case of the latter, the power of attorney shall be enclosed to the application)
• Description of your request to EOS Matrix EOOD / EOS Services EOOD
• Your preferred form of feedback on your application
